Welcome to Westwood Middle School. The following information is needed to complete the registration and enrollment of any student:
(1) Complete this online registration form.
(2) Two proofs of physical residency, ie mortgage/lease, utility bill(s), cable agreement, cell phone bill, etc
(3) Most recent copy of the student's immunization record
Any records from previously attended school(s) are helpful but not required. Westwood Middle School will request an official copy of your student's records once steps 1-3 have been completed.
